I hereby agree as a participant or as a parent or guardian of a participant that I have read and understand the terms and conditions of the London TFC Summer Camp and that further, I release owners, directors, officers, executives, committee members, employees, coaches, volunteers, other participants, agents, representatives, staff, assistances and coaches of London TFC Academy and/or FC London and/or London's Club Inc. (collectively the 'Organization') or its affiliates including but not limited to Toronto FC, Maple Leaf Sports and Entertainment, Tricar Inc., and Carvest Inc. of responsibility for any injury, personal injury, damage, property damage, expense, loss of income or loss of any kind suffered by a participant during, or as a result of any program ,activity, or event, caused in any manner whatsoever including, but not limited to, the negligence of the Organization or its affiliates. I understand that it is my sole responsibility to provide appropriate insurance coverage for participation through Ontario Soccer Registration and/or through personal insurance. Also, I agree to be bound by the Terms and Conditions of the Refund Policy. Also, by registering below, I certify that the information given in the application is true and accurate and that other than outlined in the registration form, the fitness and health of the participant is suitable for participation.
